Merck Travel Scholarship Application - (PDF document)

Travel scholarships of $1,000 USD are available for attending the symposium for attendees who complete an application and meet certain qualifications. Travel Scholarships are made possible through an unrestricted educational grant provided by Merck.

To receive a scholarship, applicant must:

1. Work and live in a country of "limited resources," including (but not limited to):
Africa
parts of Asia
Eastern Europe
Central America
parts of South America

2. Be an oncology health care professional: physician, nurse, social worker, pharmacist, dentist, dental hygienist, psychologist (if other, let us know)

3. Have a professional focus of supportive cancer care

4. Sign that you could not attend without benefit of a travel scholarship

5. Complete the application (see PDF link at top of page) and return by April 2, 2004

Applications must be received by April 2, 2004 to be considered.

Applicants will be notified via email if their application is accepted for scholarship.

Scholarship money will be awarded during the symposium.
